By making an appointment, you are agreeing to the following terms and conditions:

Free Initial Consultation

You may be offered a free initial consultation; no therapy will be provided at this time. The purpose of the initial consultation is to meet, connect and discuss your presenting issue so that both yourself and I feel a connection,for you to feel comfortable working together. This initial consultation will also allow me to evaluate and estimate if more than one session will be required on the basis of the information given and presented at the time. Although many issues can be dealt with in one session using RTT method.

Cancellations, rescheduling & refunds

If you need to reschedule a session, a minimum of 48 hours notice is required and this must be done via email. Cancellations must be done via email. No refunds shall be issued for cacellations within 48 hours of the Rapid Transformation therapy session booking. 

Sessions fees are for my time and professional expertise. They are not a guarantee of success.

If a program of several therapy sessions has been granted upon and program paid in full in advance, if you choose to discontinue your program therapy sessions, a pro rata refund will be issued after the deduction of the full standard session fee for any sessions you have attended.

Client Waiver Form

I, hereby release Melanie Arnold from any liability or claims that could be made against her, concerning my mental and or physical well being during hte work that has been outlined and agreed upon, now or in the future.

I understand that Melanie Arnold is not a physician or a psychologist. 

I give Melanie Arnold full permission to use Rapid Transformation Therapy knowing that by participating fully in the process and by including listening to my personalized recording for a minimum of 21 daysplays a fundamental role in my overall success.

I understand that although Rapid Transformation Therapy has an incredibly high success rate, Melanie Arnold cannot and does not guaratee results since my own personal success depends on many factors that Melanie Arnold does not have control over, including my willingness and desire to affect the changes inside myself.

I hereby give Melanie Arnold consent to make the audio recordings that may include my voice and respecting GHRD regulations, that Melanie Arnold has joint copyright with myself, but that all bespoke recordings that are made by Melanie Arnold retains full to Melanie Arnold.

Live in person therapy

I hereby grant Melanie Arnold permission to respectfully lift my arm, touch my shoulders, rock my head, sway my hands or tap my forehead during my physical one on one Rapid Transformation Therapy session(s) in order to facilitate any deepening processes.

I agree during an online session of Rapid Transformation Therapy to follow the invitation to rock my head gently, raise my arms.

By signin this form, I consent that Melanie Arnold will store the data as part of the reporting and documentation process, I also understand that any time Melanie Arnold may discuss aspects of my case with other colleagues keeping my full name and identity completely confidential.
